We want your visit to be as easy and comfortable as possible. Here you’ll find answers to common questions about booking, your first appointment, intake forms, insurance, direct billing, and clinic policies.

Booking & Appointments

How do I book an appointment?

You can book online through our booking system. If you cannot find a suitable appointment time, please call us at 647-482-6633 or email lisawellnessclinic@gmail.com. We may be able to help you find another available time.

What services do you currently offer?

We currently offer massage therapy, acupuncture, osteopathy, and selected wellness services. Service availability may vary by practitioner and appointment date.

What should I prepare for my first appointment?

Please arrive 5–10 minutes early to complete any outstanding intake forms and allow time for direct billing setup, if applicable.
If you plan to use insurance, please bring your insurance information and any doctor’s prescription required by your plan.

What happens during my first visit?

Your practitioner will review your health history, discuss your concerns and treatment goals, and explain the proposed treatment. You are welcome to ask questions or communicate your comfort preferences at any time.

Intake Forms

Do I need to complete an intake form?

Yes. After booking, you will receive an email asking you to complete the appropriate intake form before your appointment.
If you do not receive the form, please contact us and we can resend it.

Why do existing clients need to update their health history?

Existing clients may be asked to update their health history periodically to keep their records current and meet applicable professional requirements.
Please let your practitioner know if your health, medication, or contact information has changed.

Insurance & Direct Billing

Do you offer direct billing?

Yes. We offer direct billing for many major insurance providers.
Direct billing is provided as a courtesy and cannot be guaranteed. Eligibility, coverage limits, referral requirements, and reimbursement amounts depend on your individual insurance plan.

What information is required for direct billing?

A completed direct billing authorization form is required. Please provide accurate insurance information and bring a doctor’s prescription if your insurance provider requires one.
Some plans may require an updated prescription each year.

Can you confirm how much my insurance will cover?

Coverage varies by insurance plan. We recommend contacting your insurance provider directly to confirm:

  • Whether the service is covered

  • Your annual coverage limit

  • Whether a doctor’s prescription is required

  • Any deductible or co-payment

  • Whether direct billing is permitted

What happens if direct billing does not work?

If your claim cannot be processed or is declined, payment will be required at the clinic. We will provide an official receipt that you may submit to your insurance provider for reimbursement.
Clients are responsible for any amount not covered by their insurance plan.

Do I need a doctor’s prescription?

A prescription is not normally required to book an appointment. However, some insurance plans require one for direct billing or reimbursement. Please confirm the requirements with your insurance provider before your visit.

Clinic Policies

What is your cancellation policy?

We require at least 24 hours’ notice to cancel or reschedule an appointment.
Appointments changed or cancelled with less than 24 hours’ notice may be subject to a cancellation fee of up to the full cost of the original appointment.

Can I cancel or reschedule online?

Yes. You can normally cancel or reschedule through the online booking system when more than 24 hours remain before your appointment.
For changes within 24 hours, please contact the clinic directly.

Gift Cards

Do you offer gift cards?

Yes. Lisa Wellness gift cards can be purchased for selected massage and wellness services.
Gift cards are non-refundable, cannot be used for insurance direct billing, and may only be applied to eligible services or remaining balances according to the terms shown on the card.
